Orientation & Registration 2020-2021

We will be following CDC guidelines, which include social distancing (6 ft. apart) and wearing face masks. These registration dates may change due to any changes that may occur with LA County Department of Health Guidelines.

 

**ORIENTATION DOCUMENT SUBMISSION***

Every parent must login to their Aeries Parent Portal to access all authorizations that must be completed prior to our orientation dates below.

Orientation forms will no longer be completed via PDF Acrobat. 

 

Please be advised, prior to creating an Aeries Parent Portal, an active email account is required. Gmail account works best with our student database Aeries. Below is the tutorial/steps on how to create an email and parent portal accounts.

 

STUDENT ORIENTATION INFORMATION

 

Current LUSD students who attended LUSD elementary schools will have orientation on the dates below. All student must complete the Orientation Forms electronically. Please follow the instructions below on how to submit your orientation forms.

 

If you did not receive this information via the USPS, please contact the main office to ensure your child is enrolled at Cesar Chavez Middle School.

 

You may also click on School Locator on the right to verify your address belongs to Cesar Chavez Middle School. Should your address not belong to Cesar Chavez Middle School, it will indicate your home school.

 

If you would like to attend Cesar Chavez Middle School for this upcoming school year and your home school is not Cesar Chavez Middle School, please make sure you visit the Student Services Department at our District Office to request an intra-district permit.

 

 

Our Online Orientation is on its way and we need you to do two things:

Have an active personal account (Gmail works best)
Create or update your Aeries Parent Portal

 

 

 


STEP 1: Check your LUSD student Gmail account (ex: ssslll2222@stu.mylusd.org

Orientation Forms will need to be complete via the Aeries Parent Portal.

STEP 2: Complete all sections required

Please make sure you complete every section and submit.

STEP 3: Complete Forms Electronically

Please complete authorizations on the Aeries portal. 

 

DO NOT PRINT any paperwork. Please keep the final confirmation page and informational packet for your records. Office staff will print the confirmation page for you, and they will have it ready for both the student and parent signature during orientation in August.

STEP 4: Final Information upload

Please open the Orientation Page link.

If you are an incoming 7th grade student, you must upload your immunization record on our orientation page.

Also, if your address differs from your Aeries account, please upload your proof of address no older than 30 days.

STEP 5: Attend your Orientation Assigned Date and Time

Please make sure you arrived during your allotted time for orientation. All orientation forms and authorizations must be completed on your Aeries Parent Portal. Chavez staff will print all required documents for signature prior to your meeting with the orientation staff on the dates and times below.

As a reminder, orientation will be done virtually from this point on.

Once you create your Aeries parent account you will need the following information:

  • Student ID#
    • To request your child’s ID Number or Verification Pass Code please click here.
  • Primary Phone on file

During your Data Confirmation (Online Orientation) you will be asked to complete all sections prior to passing to the next one.

When you are confirming your data, Aeries will ask for you to print your confirmation page. PLEASE DO NOT PRINT. Cesar Chavez Middle School staff will print all necessary documents for you and your child to sign on your orientation dates in August.

On Orientation Day, you and your child will need and receive the following items:

  • Parent Identification
  • Return old textbooks
  • ONLY 6th/7th Grades: Return chromebooks checked out at Elementary Schools.
  • Receive a chromebook, if one is not checked-out.
  • Receive textbooks
  • Signatures for the documents:
    • Title I (Parent Signature)
    • Three-way Compact (Parent and Student Signature)
    • Data Confirmation Page (Parent Signature)

Orientation Process:

Step 1: Present Parent and Student ID

Document Signature and Student Locator Card

Parent and student signature will be needed to pass to the next step.

Step 2: Receive Chromebook

Receive your CCMS Chromebook.

Grade 7: Please contact your elementary to return their chromebooks prior to our orientation.

Step 3: Receive Textbooks

Receive your current grade textbooks.

Grade 8: Return your 7th grade textbooks

Grade 7: Please contact your elementary to return your textbook for 6th grade prior to our orientation.

Step 4: Take Picture for ID

 

Walk-ups for orientation will not be available at this time. We will have Counselors available for walk-up assistance in creating a Gmail and Aeries Parent Portal accounts ONLY. Parents will need to complete the Data Confirmation at home at a later time.
